it's been going on forever. food, or in-game calories need more work. your Sims can still gain weight even if they live off salad. they need to work out/jog every day to balance it out. quick meals will also make them put on weight. there's been attempts to fix it, but it's still quite noticeable with NPCs as well.

Weight related spells and/or potions for Spellcasters would be fun. I’ve looked around, and as far as I know, no one has done this yet. A little surprised Realm of Magic didn’t have them by default, to be honest. Witches in The Sims 3 could make potions that made your sims gain or lose weight without the need for mods.
